Our licensed clinical social workers will assess your needs, develop a care plan and arrange for and coordinate necessary services. JFCS care coordinators are credentialed, experienced geriatric social workers. They combine compassionate care with the objectivity and expertise necessary to help you make informed decisions.

How We Help

After a comprehensive, in-home evaluation, a care plan designed to assure your safety, independence and overall well-being is developed and implemented. Home visits and frequent phone contact allow care coordinators to evaluate current services and suggest changes in a timely manner.

We focus on prevention, which helps you stay safer and healthier for longer. JFCS offers immediate backup if your care coordinator is unavailable. Coordination of your doctor and other appointments, financial management assistance and more provide support for daily living.

If you decide you are no longer able to safely and comfortably live at home, we will present residential options that meet your needs, preferences and finances; help with arrangements to make the transition as smooth as possible; and provide ongoing care if desired.

Live independently with confidence with the ElderAlert personal emergency response system. Emergency assistance is available at the push of a button. 24 hours a day. 7 days a week.

How We Help

Trained technicians install the ElderAlert system, instruct you on how to use it, and test it monthly. The button-activated base unit connects to your telephone with a built-in battery backup. An alarm alerts you when the system is activated. You also get a button-activated waterproof, miniature emergency alert pendant or wristband that is effective up to 600 feet from the base unit. A low-battery signal contacts the response center and a technician will come and replace it. For those with no landline, a cellular service option is available as well as a mobile GPS option.

Most Holocaust survivors are widows and widowers now in their 80s and 90s. They may feel isolated, depressed, alone and anxious. Their ability to cope with the challenges of illness and aging can be complicated by the suffering, loss and deprivation experienced during the Holocaust.

How We Help

JFCS’s program for Holocaust survivors addresses the unique needs of Jewish victims of Nazi persecution through services including home-based care, counseling and help in filing claims for reparations.

Social services for Nazi victims are supported by a grant from the Conference on Jewish Material Claims Against Germany, Inc.
