We believe that everyone deserves to live a life of dignity and respect. That's why we strive to provide the best possible services for our community's most vulnerable individuals.

When an individual is no longer capable of making decisions, written and notarized power of attorney (POA) documents (no court involvement) can bestow upon JFCS the legal power of attorney to act on a ward’s behalf, according to the terms of the document.

How We Help

JFCS guardianship caseworkers act as surrogate decision-makers, always making decisions in the best interests of our wards, and based upon what the individuals’ decisions would have been if they were capable of making decisions themselves. JFCS guardianship caseworkers develop a personalized POA document for each individual, ensuring all health care directives are documented and acted upon once that person can no longer give directives on their own behalf.

JFCS guardianship caseworkers serve a diverse population and are sensitive to all religious and cultural practices. The JFCS Guardianship program follows all standards set forth by the National Guardianship Association, State, and County guidelines.

We believe it’s important to help older adults maintain their independence. That’s why we provide additional support that helps them continue to live on their own and make decisions as they see fit.

How We Help

JFCS Guardianship case workers provide support for individuals with disabilities who need help managing their benefits and planning their medical care.

In addition to helping them with the paperwork, we make sure they get to their appointments on time, assist them in arranging transportation to those appointments, and make regular wellness checks. Our fee is $75/hour.

We can help with Medical Assistance applications. We have a team of highly-trained employees who are experts in the field of helping people qualify for medical assistance. 

How We Help

We are here to provide the resources that will make it easier for you to care for your loved one, regardless of their age or disability. We coordinate services to nursing care or asset review and how to qualify for medical assistance.

How We Help

JFCS Guardianship DIrector Nicole Iole is a certified mediator with more than 10 years of experience helping families come to agreement when they have disputes. She’s available for $80/hour and can work with any situation or any party involved in your dispute.
