JF&CS staff nationally recognized as Certified Care Managers

JF&CS is proud to announce that three of our staff members have been nationally recognized as Certified Care Managers.

Pearl Averbach, Sandy Budd and Stefanie Small, Geriatric Care Coordinators and Psychotherapists at JF&CS, were recently certified through the National Academy of Certified Care Managers. This rigorous national certification provides added assurance of professional competence to perform the full range of care management tasks with excellence.

Pearl, Sandy and Stefanie have more than 50 years of combined experience in geriatric care management, also known as care coordination.

The goal of care management is to enable older adults to obtain the highest level of independence, comfort and safety consistent with their capacity and preferences for care as well as give their family member’s peace of mind.

After conducting a thorough, needs-based assessment, care coordinators will arrange a plan of care that may include:

· Monitoring in-home help
· Reviewing financial, legal or medical issues
· Provide crisis intervention
· Acting as a liaison to families and keeping families informed
· Implementing additional services as needs change
· And much more

Managing care and finding services for an aging loved one can be daunting. Individuals can turn to JF&CS when facing difficult decisions on how to care for themselves or their aging parents, no matter the distance or location.

If you know someone who could benefit from Care Coordination services, we can help. We have the resources and compassion to help you and your loved one with care-related decisions.

PS. Did you know that through our Caregiver Connection, we also provide a network of experienced caregivers for adults who require assistance to live safely and independently in their own homes? For more information, visit our website or call us at the number listed above.
